Appointment Intake Form

To help us be more efficient, please fill this form out prior to your appointment.  Please let the receptionist know at check in that the form was completed.  Thank you!!

About your visit

Payment:  Payment in full at the time of service is required.  We take Visa, MasterCard, Discover, Cash, and Care Credit.  We do take personal checks, but checks over $200 will be ran electronically and will require a drivers license.

Appointments:   If you fail to come to your appointment, or fail to give 24 hour notice for cancellation, you will be charged $26 per pet that had an appointment.  If you fail to show up for two appointments, you will be asked to find other vet care.  Our appointments are in very high demand and we need to reserve them for clients in good standing who keep appointment times.

Prescriptions:  Please call us (or request online) refills on your pets prescription a minimum of 24 hours before you need to pickup.  Many times prescriptions need to be approved by the doctor, and this gives us ample time to speak with the doctor and get the medications approved and filled.  If you choose to get your pets prescription at an online pharmacy other than ours, or at a human pharmacy, please be advised we do charge a prescription writing fee to offset the extra time involved in that process.  HOWEVER, you can skip the prescription writing fee by using our online pharmacy!  Check out the link below!

Waiting Room:  Please make sure your dog is on a leash, and your cat is in a carrier.  There is a good chance you will be sharing the waiting area with other dogs or cats.  If your pet is ill or coughing, we ask you keep your pet in the vehicle until we have a room ready for them.  **WAITING ROOM CURRENTLY CLOSED**  Please call when arriving for instructions on how to check in.
